New Patient Resources

If you are a new patient with dizziness, ear problems, or are a possible surgery candidate, your initial visit will be a comprehensive one. You will see Dr. Beal and possibly the audiologist and/or nurse for additional testing and scheduling issues. This may take up to an additional hour after Dr. Beal sees you.

Dr. Beal will do his best to see you in a timely manner and make your experience in this office pleasant, but please be prepared to take the time for us to evaluate your problem thoroughly and establish your plan of care. If you have to wait, Dr. Beal will always attempt to give you the time needed to address your concerns.

What to bring to your appointment

  •  Your insurance card
  •  Physician referral forms if required by insurance
  •  A list of current prescriptions and/or over-the-counter medications you are taking, including dose and frequency
  •  Pertinent information about your medical and surgical history
  •  Any recent x-rays or appropriate records you may have
  •  Results of audio testing
